Based on the description here: https://www.artima.com/weblogs/viewpost.jsp?thread=164293 which matches the behavior of the `diff`/`patch` tools. For all of the tests created or modified, behavior was verified to match that of `diff`/`patch`. Fixes kpdecker#296
