Not able to reapply for github student pack after expiring and my college elft

When i am tring to reapply for student pack i am clikcing on submit and getting pack to that page

from here

then

i fill this and click appy then i get reverted back to here with empty state and no error

liek this

please guide me

Usually this issue is solved by submitting the application using another device, such as a smartphone or another PC.

These users solved it by using the phone: #162877, #168688, #175904

If none of the above works, I recommend you contact support by opening a ticket here, select "Other" and explain your problem.
